For violations of their WASTEWATER CONTRIBUTION PERMIT (Permit No. 001-03-96). Violations included discharging wastewater with Hydrocarbon Oil and Grease concentrations exceeding the 25 mg/L permit limitation, a slug discharge, improper operation of the oil/water separator, and reporting violations. Specific sample results for Hydrocarbon Oil and Grease were as follows:

PUBLIC NOTICE
This notice is intended to inform the residents of Whitley County, Kentucky, that the Whitley County Fiscal Court is in the process of closing out the Whitley County Independence House (Recovery KY) Projects, CDBG 11-089 and CDBG 12-081. The projects were funded in part by the Kentucky Community Development Block Grant Program. A public hearing will be held at the Whitley County Courthouse, in the Fiscal Court Room, on Wednesday, June 18, 2014, at 11:00 a.m., Local Time. The purpose of this hearing is to review past use of funds and program performance. If there are any questions or comments about the project, please direct them to the following:
