REQUEST FOR QUALIFICATIONS
The Whitley County Water District will retain a qualified engineering firm to provide the necessary planning, design, inspection, and other engineering services for a proposed Water System Improvements Project (WX21235434). The Owner will accept Statements of Qualifications from engineering firms interested in working on this project, until Tuesday, March 24, 2015, at 4:00 p.m. An RFQ packet containing information about the project and criteria, which will be used to select the consulting firm, may be obtained by contacting Andy Meadors or Sandy Smith at 606-549-3600 or at the District Office, 19 South Hwy 25W, Williamsburg, KY 40769. The Statement of Qualifications will be considered by a selection committee appointed by the Owner. The Owner reserves the right to reject any and all submittals.
This solicitation for “Statements of Qualifications” is being conducted to fulfill funding agency procurement requirements. Attention of respondents to this RFQ is called to the following requirements: Section 3 of the Housing and Urban Development Act of 1968; Section 109 of the Housing and Community Development Act of 1974; Title VI of the Civil Rights Act of 1964; Executive Order 11246; Certification of Non-segregated Facilities; and all other applicable federal and state laws and regulations. No person shall be excluded from participation in, denied benefits of, or subjected to discrimination in the implementation of this project on the grounds of race, color, national origin, sex, or handicapped status. Hwy. 25W, Williamsburg, Kentucky 40769, has applied for a permit for a surface coal mining operation affecting 234.80 acres of surface disturbance and 126.70 acres of auger mining located on Buffalo Creek of Clear Fork.
The proposed operation is approximately 2.5 miles south of Pearl, Kentucky and located approximately 0.8 miles from the intersection of General Lowe Road and Highway 190. The latitude is 36° 35’ 39”. The longitude is 83° 56’ 15”.
The proposed permit is located on the Eagan USGS 7 ½ minute quadrangle map. The operation will use the combination of surface and auger mining. The property owners affected by the surface and auger mining proposed in this permit are Lynn Hamblin, Norma Lowe, Dallas Shackelford, Jackie Hatfield, Ester Partin, Phylliss Nichlos, Pamela K Reynolds, Alesha Nicole Partin Simmerman, Brandon Partin, Mountainside Coal Company Inc. NOTICE OF BLASTING SCHEDULE
This is notice that FrazierNantz, LLC. Of 541 Blakes Fork Rd. Williamsburg, Ky. 40769, Permit #918-0471 will be blasting at their mine site located approximately 0.2 miles west from KY 1064 and Spring Town Road Junction. Blasting will be done daily. No blasting will be conducted before sunrise or after sunset. At least 10 minutes before the blast, Company personnel will control access to the area. Before each blast is detonated the following type(s) of audible warning will be given: WARNING SIGNAL – A one-(1) minute series of long blasts of a siren, five (5) minutes prior to the blast signal. BLAST SIGNAL – A series of short blasts of a siren, one (1) minute prior to the shot. The all-clear signal(s) after blasting will be: A prolonged blast of a siren, following the inspection of the blast area. Blasting will not be conducted at times different from those given above except in emergency situations where rain, lightning, other atmospheric conditions, or operator or public safety requires unscheduled detonation. Prior to these detonations, the following audible warning will be given: . A one-(1) minute series of long blasts of a siren, five (5) minutes prior to the blast signal. BLAST SIGNAL – A series of short blasts of a siren, one (1) minute prior to the shot. The all-clear signal(s) after blasting will be: A prolonged blast of a siren, following the inspection of the blast area Also prior to detonations in emergency situations the permittee, using audible signals, shall notify all persons within one half (1/2) mile of the blasting site. All roads leading to or located within 1000’ feet of the blast area will be blocked by flagman a minimum of 1000’ feet from the blast prior to detonation and remain closed until the all clear is given.
The “Blasting Schedule” will be published in The News Journal, the newspaper with the largest circulation in Whitley county at least 10 days, but not more than 30 days, before beginning a blasting program that will be conducted From March 2015 to March 2016.
The “Blasting Schedule” will be republished and redistributed at least every twelve (12) months. The “Blasting Schedule” shall be revised, republished, and redistributed at least ten (10) days, but not more than thirty (30) days prior to blasting whenever the area covered by the schedule changes, the actual time periods for blasting significantly differ from those identified in the prior announcement, or the permittee changes the types or patterns of warning or all clear signals identified in the prior schedule.
Copies of the “Blasting Schedule” will be sent to each person within the permit area and each person who resides or regularly works within one-half (1/2) mile of the permit area at least ten (10) days, but not more than thirty (30) days, before beginning the blasting program.

BRIDGE ABUTMENTS

Bids will be accepted for the concrete bridge abutments for a 33’x16’ bridge on Bethel Road.
ALL BIDDERS MUST BE PRE-QUALIFIED through the Kentucky Department of Transportation.
Detailed plans are available at the Whitley County Judge Executive’s office. As the steel deck has already been bid and accepted the contractor will disregard that portion of the plans except where it pertains to the abutments.
Contractor shall provide the Whitley County Fiscal Court proof of Liability Insurance and a bid bond equal to ten percent (10%) of the total bid.

County road department personnel will be responsible for demolition of existing structure.
The county will procure all necessary permits prior to installation. Whitley County Road Department will be responsible for backfilling both ends of the bridge to road grade.
Contractor will not begin work until given the release to start work. The county will give the contractor a notice to proceed after receiving the permitting and the KYDOH Memorandum of Agreement. Once notified to begin the contractor will be given 30 working days to complete the project.
