I have been appointed Warning Order Attorney by the Knox Circuit Court in the above styled action of which you have been named as a defendant. My chief duty as Warning Order Attorney is to inform you of this proceeding. A copy of the complaint can be seen at the Law Office of Millward & Castle, 114 Court Square, Barbourville, Kentucky 40906.

It is imperative that you contact me at the above address and/or telephone number as soon as possible regarding this matter. Should you neglect to assert any legal defenses to this claim, a judgment could be entered against you.

I ask that you reply within twenty (20) days of this notice. If I do not receive your response by that time, I will proceed with my report to the court. If no answer is filed with the Knox Circuit Court and the attorney for the petitioner within fifty (50) days from January 12, 2022, the court may grant the relief requested in the complaint.

INVITATION FOR BID #2022-01-02

The Housing Authority of Williamsburg (HAW) will receive sealed bids for LAWN CARE services from qualified contractors until 1:30 pm local prevailing time on February 22, 2022 at the Housing Authority of Williamsburg, 600 Brush Arbor Rd, Williamsburg, KY 40769 at which time and place bids will be publicly opened and read aloud. BIDS RECEIVED AFTER THE STIPULATED DATE AND TIME WILL NOT BE CONSIDERED. The HAW will not be held responsible for late delivery of bids by the U.S. Postal Service or any other means. THE ENVELOPE CONTAINING THE SEALED BID shall be marked as follows:

Lawn Care services will be conducted at five (5) separate locations of the Housing Authority of Williamsburg’s property in Williamsburg, KY.

Specifications of Work:

1. a) Contractor shall provide all materials, labor, fuel, equipment, bags for removal of cuttings and trash, permits, and any items needed to complete the lawn care services.
2. b) All lawns areas of the HAW property shall be machine mowed and machine edged.
3. c) Trimming around all buildings, sidewalks, landscaping, fences, playgrounds, gazebos, and paved surfaces around the perimeter of the HAW property.
4. d) Clippings are to be blown off sidewalks, porches, and landscaping. Clippings cannot be blown toward any vehicles.
5. e) Removal of debris from property before departure.
6. f) Consideration must be given to weather conditions so that the lawn or landscaping are not damaged.
7. g) Clean gutters at all HAW sites, once in April and once in October.
8. h) All HAW landscaped areas are to be kept weed-free, free of leaves and free of all debris.
9. i) All HAW landscaped areas shrubbery is to be trimmed at the beginning and end of mowing season.
10. j) Leaves are to be mowed and removed.

Frequency:

• Lawn Care services will be provided every 7 to 10 days, during daylight hours, beginning April 1, 2022, through October 31, 2022.
• This Lawn Care Bid is with option to be extended up to 5 years per Executive Director.

Contractor Responsibility:

1. a) Contractor is required to provide proof of liability insurance and workers compensation as well as agrees to maintain for the duration of the life of the contract.
2. b) Contractor will be responsible for repair or replacement of any damage to property belonging to the Housing Authority of Williamsburg or tenant property caused by equipment used by contractor in mowing, edging, trimming, or blowing, this includes tenant vehicles.
3. c) Care shall be taken to avoid damage or injury to trees and shrubs and building downspouts, siding, etc. when working to trim and edge the properties.
4. d) Contractor staff must wear identification badge or uniform while on HAW properties.
5. e) Contractor will take all steps to protect the public from injury.
6. f) Contractor is required to provide three (3) references.
7. g) Contractor must be in compliance with HUD Maintenance Wage Rates.
8. h) Contractor shall enforce proper discipline and good order among Contractor employees and subcontractors at all times.
9. i) Contractor’s, subcontractor’s and their employee vehicles and delivery trucks shall be parked so as not to hinder street traffic, or impact adjacent resident’s use of their property. STRICTLY PROHIBITED is parking on sidewalks and green ways, as well as driving over curbs.
10. j) Contractor shall submit weekly invoices to HAW with reference to property name, date and itemized work performed including but not limited to costs per line item resulting in total requested payment, which shall compare to Form of Bid costs. Checks will be printed bi-weekly.
11. k) Under HUD Section 3 the HAW encourages contractors to hire Section 3 residents.

Lawn Care Service Bid Packets can be picked up at 600 Brush Arbor Rd., Williamsburg, KY 40769 Monday through Thursday from 8:00am to 5:30pm.

Prebid conference including a tour of sites for interested contractors will be February 14, 2022, at 10:00am.
